Please provide the following documentation for each student:

• Official Immunization Record with all shots completed per Utah law

• Certified Birth Certificate (we will make a copy for our records)

• Valid picture ID to verify guardianship

• Proof of Residency in guardian's name - 2 items (click here for Proof of Residency Requirements)

PLEASE NOTE:  You cannot use the same picture ID to prove guardianship and also to show proof of residency. 

Please print and fill out these documents completely and sign where appropriate.

7. If your student has medical needs, you can find the authorization forms you'll need here. 

Please bring all requested items to the school office for processing. Registration should be completed at least one day before the student's first day.
